Preparation

What documents might you need?

Get the right proof ready before you start, and only upload it through an official application.

Basic details

The application typically asks for your legal name, date of birth, home address, and the last four digits of your Social Security number or a Tribal ID number. Enter details as they appear on your records.

Proof of a program or income

If automatic checks cannot confirm eligibility, you may be asked for a current benefit letter or another official record showing program participation. For income, USAC lists accepted records such as a tax return or consecutive pay stubs. Requirements depend on the reason you qualify.

If something does not match

USAC may request proof of identity, address, or household status. Follow the exact document request shown in the official application. Do not send identity documents through social media, this guide, or an unsolicited link.

Check the official source for current rules and figures.
