Get the right proof ready without sending it to this guide.
- The verifier will tell you exactly which document is needed, if any.
- Program proof should identify the person, program, issuer, and a recent date.
- Income proof may include a prior-year return or three consecutive months of pay stubs.
Identity and address
If an automated check cannot verify you, USAC may request a current ID, birth certificate, SSN or Tribal ID proof, or address evidence. A driver's license or government ID, utility bill, lease, W-2, tax return, or a map showing your physical location may be accepted for address, depending on the request.
Program participation
A benefit award letter, statement, verification letter, or screenshot from a benefits portal may work. It should show your name or your dependent's name, the program, issuing agency, and a recent issue date or future expiration.
Household income
USAC lists a prior-year tax return, current annual income statement, Social Security benefits statement, unemployment statement, or official income records. Three consecutive months of dated pay stubs may also demonstrate income. Use the exact documents requested.
Name or address does not match?
Compare your entries with official records, including spelling, suffixes, and apartment number. Read the verifier's exact request, then upload a legible document through the official application. USAC can explain the proof needed.
No standard street address?
The online application can use a mapping tool for a physical location; a map with location coordinates may be accepted. A mailing address and physical location serve different purposes. Ask USAC for help if you lack a permanent address.
Share records safely
Never send ID photos, benefits letters, Social Security numbers, or application codes to this guide or to public social posts. Use the official application or instructions from USAC/SafeLink.
